Pre-Participation Examination (PPE) Process

For All Freshman, Transfers, First Year Student-Athletes, and Returning Students

Before you are allowed to participate in any team activities (i.e. practices, competitions, conditioning and/or weight training) you must be medically cleared and have on file in the athletic training room all necessary paperwork.
In order to be medically cleared all student athletes must complete the following steps:

1.    All prospective athletes must first speak with the coach of the team they wish to join.  The coach will then inform them of how to contact an LVC sports medicine staff member.
2.    First year students and transfers are required to complete the Shroyer Health Center physical form which includes completing a physical examination with your family physician.  This form may be used for both the school required physical and the athletic participation physical. It must be turned in to the Health Center and a copy of page 2 sent to the Athletic Training room.  Important Note: In regards to your physical, the NCAA requires that all first time student-athletes must have a Pre-Participation Examination (PPE) completed no more than 6 months prior to their first participation, i.e. practice, competition, or out-of season conditioning activities (rule 17.1.5 p 124-125 in the NCAA D-III Manual).  If you are unsure of your sport’s first participation date, please contact your head coach.
3.    Returning athletes are to get a physical from their family physician.  If the athlete does not wish to see their own physician, or if they do not have a family physician, they may contact Shroyer Health Center about obtaining a physical locally.  Athletes should be advised it is not free and has to be scheduled.
4.    After completing the physician exam, prospective athletes will contact the LVC sports medicine staff to arrange a date to have an ATR screening/paperwork review.
5.    Athletes will meet with LVC sports medicine staff to complete ATR screening, turn in all paperwork including the athletic clearance medical history form signed by doctor and have paperwork reviewed.   
6.    Athletes will arrange to take the ImPACT concussion test with a member of the LVC sports medicine staff.  
7.    An email will be sent to the Head Coach, Athletics Secretary, Athletic Director, Assistant Athletic Director, and Equipment Manager notifying them that the athlete is cleared for participation.

Athletes are not cleared until all the above steps have been met and an email has been sent. Completing a doctor physical does not constitute clearance.  Athletes must meet with a member of LVC Sports Medicine Staff before they are considered medically cleared!
